Shiver by Maggie Stiefvater

Sam lives in two worlds -- during the winter he hovers around Grace's house as a wolf and when the weather thaws, he transforms into a human. For years Grace has been watching her yellow-eyed wolf. She's drawn to him in a way she can't explain, but it soon becomes crystal clear when the two finally meet face to face.

As the two get to know each other and begin to really fall in love, they must deal with the fact that Sam is a werewolf and his time as Sam is borrowed. As werewolves age, they live mostly as werewolves and less as humans. Sam knows that his last summer as a human is coming soon. It's up to them to fight for more time together before Sam loses himself and Grace forever.
