Scenario 1: They're BFFs

So are you dating one of those slightly metro boys who's friends with tons of girls? Then we wouldn't worry if his ex-crush happens to also be his BFF! Being friends with her is obviously not out of the norm for him. And even if he doesn't have lots of girls he's "just friends" with? If something were meant to happen, it would've happened by now! There's a reason why he's with you and not her. (Haven't you had a crush on a BGF at some point?). If you're really buggin', don't be afraid to talk with your BF about it. Just be careful about your phrasing: Start by saying you admire the closeness he has with BFF Girl, and you'd like to establish that same rapport.
